1. Nursing care plan of Cholecystitis are :-

Nursing Assessment :-

- Assess the vital signs of the patient

- Assess the health history of the patient

- Assess the pain status related to inflammation and infection

- Assess the nutritional status of the patient

- Assess the knowledge of patient about disease condition and self care .

1. Nursing diagnosis :-

- Acute pain related to inflammation ,edema , distension of gall bladder .

Nursing interventions are:-

- Assess the pain using visual analogue scale

- Administration of analgesics as prescribed to relieve pain

- Help patient to turn , cough ,breathe deeply and ambulate as indicated .

- Reassess the pain after administration of medications

2. Nursing diagnosis :-

Ineffective breathing pattern related to pain

Nursing interventions are :-

- advice patient to take deep breath and cough every to expand lungs fully

- promote ambulation

- monitor those patients closely who have pre existing pulmonary disease for respiratory problems

- provide semi -fowlers position

- administration of oxygen of prescribed

3. Imbalanced nutrition ,less than body requirement related to reduced intake and increased metabolic demand .

Nursing interventions are :-

- encourage a patient to eat a diet low in fat , high in protein and carbohydrates.

- small frequent diet , soft diets

- increased fluid intake

- more fiber intake

4. Nursing diagnosis :-

- Impaired skin integrity related to poor nutrition ,bed rest , multiple drains .

Nursing interventions are:-

- providing back care and sponge to avoid bed sores

- turning the patient every two hourly

- providing a high protein , vitamins C containing food to promote healing

- monitor for signs of infection like redness , inflammation, temperature , discharge etc

- if drains are present ,keep the drains below waist level to drain effectively with the help of gravity

- change dressing if any frequently using ointment to protect skin from irritation .

- place drainage bag in patients pocket when ambulation

2. Discharge planning :-

- teach patient about medications and their actions

- instruct patient to report to physician symptoms of dark urine , pale stool , pruritus , signs of infection etc

- instruct patient about drainage care if present and when to report doctors like changes in the colour of or amount of drainage

- emphasize importance of keeping follow up appointments
