Desired outcome:
Client will; FAMILY COPYING, begin to express feelings freely and appropriately, identify internal and external sources to deal with the situation ..
Intervention:
1) Assess causes and contributing factors of dysfunctional family system :assist patient to define them
2) Acknowledge your feelings about the family and situation
3) Approach family with warmth.respect and support
4) Reflect family emotions to clarify these feelings: ( 'you must find it very frightening....')
5) Involve family members in patient care conference, wherever appropriate
6) Increase the self esteem of the individual family members
7) Assist family members to reorganize the roles at home and set priorities to maintain family integrity and to reduce stress
Implementation and Evaluation are last two steps of nursing process.
Discharge planning: a check list should contain following points
-patient's understands why she/he takes the medication,time,time dose
- next appointment date and time
-name and phone numbers of the family members
-patient's living arrangement
-need an eligibility for financial aid
As a part of Discharge planning family has to receive education regarding the improvement of communication with the client,avoid stress full situation in the family and importance of follow up.
