Date - 08/02/2020

Time - 8.46pm

Situation - Mr. M has sudden onset of radiating chest pain and shortness of breath

Background - He has a history of MI last year and his vitals are HR- 135bpm, RR - 26bpm, BP - 160/100mmhg. He is on oxygen 5liters by face mask with a saturation of 86%

Assessment - I think we must do the assessment for MI as there are high chances for it .

Recommendations - I need the physician to come and evaluate the patient , meanwhile i will ask for 12 lead ECG
