When a person accessing services makes a decision that impacts the families, carers and/or significant other, what course of action should the support worker follow?

How can the support worker assist the person accessing services to choose the location of their end-of-life care?

Support workers: they provide a key role in ensuring and delivering high quality care andsupport workers are closest to patients and care and are therefor best placed to lead development and deliver successful changes, the support workers are the one delivering the basic fundamental care needs and through this develop a close professional working relationship with patients and their families, the support workers follow the action is they access the needs of the clients by conducting interviews and individual and their families and then they report they assessment and montiorin the mediacl conditions and recreational activites with clients and then provides information and support to the families of those within the care and provides companionship. 2)the support workers assist the person accessing service to choose the location of their end of life care, it provided in a range of setting people who are in end of life must be able to move freely between the places to meet their medical care and support needs and it is ususally best provided in close proximity to the clients local environment and community and it is provided in the environment of the person who is accessing the services choice and also there needs depending on the prefers hospital and some prefers the home , or purpose built hospice whatever they needs then the support workers assist them in accessing such services.
