A diverticular disease characterized by the formation of the sac-like structure in weaken intestinal wall. Which can be a pea size to very large size and contain fluids, waste products, blood, and air. Diverticulosis is the plural form of diverticular.
It commonly does not have signs and symptoms but some people may have pain in the abdominal region, cramping of belly, constipation, and diarrhea.

Intervention - Give psychological support and diversional therapy
Administer Analgesic for pain relief
Nursing Diagnosis 2 - decrease frequency of defecation related to intestinal factors as evidenced by hard stool.
Intervention - Give laxative drugs to the patient
Increase water intake of the patient
Encourage mobility
Acute ischemic right-sided weakness is due to an ischemic stroke in the brain. In this disease, the patient can not move his/her right side limbs which makes the patient bedridden.

Intervention - Assess the skin condition of the patient
Change position frequently
Provide air, water or nimbus bed
