Answer: Query process can be improved by enhancing the documentation by the physician about the health of the patients and their records. Their should be proper coordination between physician and the coding professionals, because without this coordination the query process becomes difficult. There should be better understanding between the physician and the coder. The early feedback can improve the functioning of the query process.

Sometimes when the cost of this process is too high, hospitals or physicians does not ready to bear the cost and they quit. The main aim of the query process is to obtain the accuracy, integrity in the query process where the information should be proper.
