Not covering of preexisting condition, increasing the cost of health care, the gap in the health care services, problems in stakeholders and vendors. Electronic health record has to be redesigned in order to improve the process of outcome of disease and health. The redesign of EMR should be based on analyzing of current workflows. The new design should improve the efficiency of the workflow of the organization. It increases the care coordination at all levels and enhances the quality and safety of health care.
