Reports are to kept confidentially and disclosed only to the right person after confirming. Any doubts in patient identification should make the staff not to release the report via phone and inform gently and genuinely to come and collect the report in person or as per the protocol.Generally in these situations not disclosing is the best way to avoid any consequences

It is a duty to confirm the identity and not pass the work to other staff

Releasing the requested information if done to the right person is not a concern but if released to the wrong person can lead to legal issues.

Calling the lab persons also needs proper disposal of patient identity
