For example,

• A patient is obese and has cardiovascular disease. The nurse perform assessment such as checking vital signs, checking for the cardiac output.
• After the diagnosis the nurse find that the patient is suffering from ischemia.
• The nurse provide counselling to the patient and discharge plan as by explaining patient to adopt healthy lifestyle.
