it is the conscientous use of the best evidence in combination with clinician expertise and patient preferences and values in clinicical decision making.The expectation for high quality,cost-effective care and the rapid for of easily accessible knowledge in a competing health care marketing place have driven the need for the daily use of evidence to improve point of service care .Regulatory and accrediting agencies now require that practice be based on evidence .All of these factors have led to the need for all health care providers to gain knowledge and skill in EBP.
There are five steps in the EBP process.
Step1-Step 1 of the process is asking a clinical question in a PICOT format .Formulating the clinical question is the most important and the most challenging step in the EBP Process.A clinical question that is searchable and answerable creates the context for integrating research findings,clinical judgement and patient preferences.Inaddition the question drives the search strategy and the type of evidence required to answer it.
Step 2 of the EBP process is an efficient search for and collection of evidence based on the question .THe question directs the clinician to the databases that are the most appropriate.The search begins with the strongest external evidence to answer the question .Preappraised evidence such as systematic reviews and evidence-based guidelines are appropriate time-saving resources in the EBP process.Systematic reviews of randomized controlled trials are considered the strongest level of evidence to answer questions about interventions .However limited number of systematic reviews are available to answer the many clinical questions,Inaddition systemic reviews or meta-analysis may not always be the most appropriate to all clinically meaningful questions.Evidence-based guidelines and reviews are an appropriate time-saving resource in the EBP process.
Step 3 of the the EBP processis critically appraising and synthesizing studies found in the search .A successful critical appraisal process focus on three essential questions.Are the results of the study valid?What are the results?Are the findings clinically relevant to my patients?The purpose of critical appraisal is to determine the value of the research to practice and not only to examine the flaws of the study Clinicians must determine the strength of the evidence and synthesize these findings related to the clinical question to conclude what is the best practice.
Step 4 of the EBP process maybe different depending on the strength and breadth of the evidence to answer the question.Recommendation from sufficient ,strong evidence can be implemented into practice in combination with clinicians expertise and patient preferences.Clinical judgement will influence how patient preferences and values are assessed ,integrated and entered into the decision -making process.Clinical judgement will influence how patient preferences and values are assessed ,integrated and entered into the decision-making process.Without sufficient evidence ,the fouth step would be to generate data to answer the question .Data can be gathered either through outcome management initiatives or through the conduct of rigorous research.Clinicians need to partner with experts in research to achieve success in these endeavours.In Step5 of the EBP process is evaluation of identified outcomes in the clinical setting .Outcomes must match the clinical project that has been implemented.
To implement EBP nurses must continiously seek scientific evidence that supports the care that they provide.The incorporation of evidence should be balanced with clinical expertise and should take into account the patients unique circumstances and preferences. EBP closes the gap between research and practice ,providing more reliable and predictable care than that which is based on tradition ,opinion and trial and error.
