You receive a post-transfusion specimen. You notice hemolysis in the patient plasma. What should you do next?

If transfusion is going on immediately stop transfusion of blood.

Maintain iv access (do not flush the previous iv cannula use another access)

Inform to doctor immediately.

Administer medication as advised by doctor like anti histamine, corticosteroids.... Etc

Also observe sign of haematuria, pt may need dialysis to remove hemolysed RBC's from circulation.

Send the blood bag with sample of blood and urine and fill a form of transfusion reaction signed by doctor and also note signs and symptoms observed.
