An anecdotal note should contain the main point ts clearly and briefly in using all the details

12/31 0900 Patient X complained that the LVN is administering incorrect dose of human insulin ,on rechecking the dose the LVN admitted that the patient is correct .

# As a RN , I need to solve this issue and it's of great concern to me as it's about patients life . I will have a meeting with the LVN and ask her about what is the problem ,is she not comfortable or she doesn't not how to work etc . After knowing the problem she will be given guidance on how to work and will be given a chance to improve . If she does not improves then action will be taken against her .

# While conducting a evaluation meeting with an employee we should assess the improvements that have been made by the staff on advice and how is she or he working or providing her /his services to the patient's .
