Patients who require medical or surgical intervention are referred to hospital-affiliated clinics or physicians' offices, as appropriate. These provider sites have an arrangement with the health department and receive reimbursement on a sliding scale, ac- cording to a prearranged fee schedule. Eighteen-year-old Shirley Brown was seen by a nurse practitioner (NP) at a health depart- ment clinic. She was referred to a private practice gynecologist, who diagnosed severe dys- menorrhea. Ms. Brown was treated with a regimen of medications. Several months later, she returned to the health department clinic complaining of the same problem. The NP explained that surgery might be necessary to correct her problem. Ms. Brown returned to the gynecolo gist, telling him that the NP had discussed the surgical option. The gynecologist told her that the surgical option was a last resort. She was so distressed by the severe dysmenorrhea that she told the gynecologist to do whatever was necessary to make her feel better. Ms. Brown was admitted to the hospital and underwent a hysterectomy. Only after recov ering from the surgery did she grasp the meaning of the procedure and that, as a result, she was unable to bear children. The realization caused her to become extremely distraught. She went back to the NP, with whom she had developed good rapport, to complain about her lack of understanding of the implications of surgical treatment. Questions 1. 1. All the healthcare workers working in a clinic , physician offices ,hospital etc are responsible to provide detailed information regarding the treatment and procedures which are needed and their good and side effects including complications in a language understood by the patient . And then only an informed written consent from the patient infront of the witness is taken .

2. Everyone is equally responsible in a health care area for taking an informed consent . It's a medical malpractice if we don't take consent or don't provide information regarding the treatment and procedures . It comes under the role of every health care worker that before doing a procedure an informed consent has to be taken after providing information about the same . For the physician and nurse it's a must come responsibility .

3. Patients age is very important in taking consent . Those who are 18years and above can give their consents themselves while those who are less than 18years , parents or their guardians should give the consent . In this case the patient has given the consent because she is 18 years of age.

4. To avoid such situations in future , the hospital's must a series of steps which will ensure each and every process from the entry of the patient to the hospital to the admission . The hospital must assign healthcare workers with definite actions to ensure safe and secure health care practices.
