1.The following patient education is needed for a patient who is having constipation

  • Encourage to increase the fluid content,this can soften the hard stools
  • Increase fibre rich food ,this increases the volume of the stool
  • Encourage to eat three full meals I the right way
  • Exercise can improve gastric motility
  • To go for relaxation techniques to relieve stress
  • Encourage patient to take laxatives as per order

2.The method to collect fecal occult blood are

  • The patient requires to produce three small quantity stool sample
  • Can be done whenever the patient passes stool
  • Inform patient to avoid foods like carrot ,grapefruit,broccoli ,redmeat, painkillers
  • Take the stool sample one after the other day and send it yo laboratory after each sample
  • Inform patient to collect the stool at the tip of the applicator and place it in the container

3.The following teaching has to be done to the patient about self catheterization

  • Encourage patient to wash hands using antiseptic solution or with soap and water
  • Collect all the equipment needed to drain the urine (catheter to insert and drain ,lubricant to prevent irritation and injury while insertion,clean wipes to clean perineal area before catheterization ,urine container
  • encourage to wash the perineal area,then lubricate the tube ,stand in front of a toilet and insert the tube towards gravitation and drain the urine .If not in toilet use a urine container to collect urine
  • Measure and note the colour ,odour of the urine if advised.
