Ans. Description of procedures removal of tonsil by the surgical procedure
Indication of tonsillectomy = lymphoid tissue helps prevent invasion of organism .
Indication of surgical removal 1. Recurrent tonsillitis or otitis media
2. Enlargement that interferes with breathing or swallowing contraindications for removal
- occasional infections that resolve rapidly
- cleft palate, hemophilia, or debilitating illness
3.clinical findings postoperative
- post surgical hemorrhage first 24 hours caused by trauma to vascular site, 5 to 10 days after surgery caused by sloughing of tissue
Signs of hemorrhage frequent swallowing bright red blood in vomitus, restlessness increased pulse rate, pallor
Preoperative care vital signs and physical examination and all investigation
Intra operative care assess vital signs suctioning, position, prevent aspirations and NBM (nill by mouth )
4.analysis
- presence of bleeding, frequent swallowing
- presence and extent of pain
- swallowing ability
5.planning/implantation
- keep positioned on side to promote drainage of secretion, elevate headof bed to limit edema
- offer cool liquid that are not red in color or thick, gelatin allowed if not red
- ask child to talk, provide assurance that it is possible
- Apply ice collar for pain
-use pain rating scale and administer prescribed medication
- during initial postoperative days, offer soft foods, avoid crisp food e. g. bacon, pretzels chips that are interrupt suture line
Evaluation /outcomes - maintains patent airway
- manages respiratory secretions
- reports maintain pain
- maintains fliud and nutritional status
