In the SDLC (Software Development Life Cycle), the requirement phase or "requirement gathering" phase is uder to gather and analyse the information and after the gathering and analysis is done, a SRS document is created and called as the output from the "requirement" phase.

=> The answer is SRS Document.

Now the output of the "requirement" phase (SRS Document) acts as the input for the "design" phase . In the design phase the bacisc or general logical design which was created in the "requirement" phase gets converted to a physical design and a design document is created and called as the output from the "design" phase.

=> The answer is Design Document.
