d) A premature newborn presents with yellowing of the skin. What urine value would you expect to be abnormal? What is the normal ailment associated with this issue and how is it treated?
The normal ailments associated with this issue may be Neonatal jundice.
-In infant there occurs breakdown of the fetal hemoglobin as it is replaced with adult hemoglobin.
- due to this breakdown unconjugated bilirubin is formed. This unconjugated bilirubin has to be metabolized in the liver to conjugated bilirubin. But in case of infant the liver is not mature to conjugate such large amount of unconjugated bilirubin.
- so this bilirubin come to blood plasma and some get excreted by kidney through urine.
- the urine bilirubin level increase above normal level.
- treatment: -
- neonatal jundice is treated generally by phototherapy.
- Babies with neonatal jundice is treated by coloured light called phototherapy, which works by changing trans-bilirubin into the water soluble cis-bilirubin isomer.
- generally phototherapy is started when plasma bilirubin level exceeds 20 mg/dl
Hopefully it helps thanks
