The following questions can be included in the questionnaire:

1. At what time do you sleep at night and what time do you wake up in the morning?

The rationale to ask this question is to know how long does the respondent sleeps, i.e., what amount of sleep he/she gets.

2. Do you sleep and wake up at nearly the same timings everyday?

This question would help to know whether the sleeping patterns of the respondent are consistent or not.

3. Do you wake up in the middle of the night without common reason (like thirst or to use the washroom)? If yes, how often?

The rationale for this question is to know whether the respondent has good continuous sleep or has frequent breaks in the sleep.

4. Do you take a lot of time to sleep at night (more than 30-45 mins)? If yes, how much more?

The rationale for asking this question is to know if the respondent has any problem in being able to sleep at night.

5. Do you experience a lot of nightmares or lucid dreaming in your sleep?

The rationale for asking this question is to know if the respondent has been facing sleep disturbances.

6. Do you feel sleepy during the day? If yes, how often?

The rationale for asking this question is to know whether the amount of sleep the respondent is getting is sufficient for them or not.

7. Do you require any substance in order to be able to sleep at night?

This question will help us know if the respondent has any difficulty in sleeping because of which he/she is dependent on any substance like sleeping pills or alcohol.

8. Do you expose your eyes to light screen (like television, mobile or laptop) before going to bed?

The rationale for this question is to see if extra exposure to screen is the cause for delay in sleep or sleep problems.

9. Are you aware of whether you sleep walk or sleep talk?

This question would help to know if the respondent is suffering form problems like sleep walking or sleep talking which would also shed light on whether the respondent has problem with REM sleep.

10. Do you feel the urge to frequently urinate or the issue of bed-wetting?

The rationale for this question is to know if there is some underlying problem or issues with sleep.
