Internal radiation therapy (brachytherapy) allows a higher dose of radiation in a smaller area that might be possible with external radiation treatment in lower dose of radiation. Higher dose of radiation means more side effect and less dose of radiation means less side effect.
Thus, it means external radiation therapy is less harmful because it has less side effects due to it's requirement of less concentration of radiation whereas internal radiation therapy is more harmful because it has more side effects due it's requirement of high concentration of radiation.
