Risk is the term which shows uncertainity. Risk is something which shows probability of any loss, injury, liability etc. The example of Risk is when you decide that you want to do swimming in Cold water for an hour. then there is risk that you will get sick like attract fever.
Outcomes are actual result of a process or act. Outcome is the final consequence you face in a process. in above example- the main outcome is that you become sick due to swimming in cold water.

Causes are those points which are the main reason or roots of happening something. Causes are something or someone that produces an effect.
in above example- the cold water was the cause that you become sick.
another example of cause. a person was driving a car and car become stopped in the middle of the road because fuel tank got empty. the cause in this situation is no Fuel in the car.
the implication of confusing them is that it affect outcome and poor decision making. every person find cause and risks to make decision but when they confuse them then their analysis and decision making become poor. the confusion is very evident in healthcare industry where doctors were not able to distinguish between cause and risk and make poor decision in diagnosis.
