Index case is nothing but the primary case which means the first patient documented in the proccess of epidimiology investigation . It may also be refferred as the furst syndrome to decide whether the patient is only the first person effected with it .

Basically index case may not give the whole information of the disease and related but it definitely hints us that there is something going on and it may spread not necessarily contagious but it does give us awareness . And that there is a outbreak of it .

Primary case tells only about the human to human outbreak or transmission . It tells us that how this outbreak even started , and how to stop it . So basically index or primary terms refers to the first person got infected.
