PCMH i.e. patient-centered medical homes is a medical home in which the primary care physician coordinates with the patient treatment and they ensure that their patient receives the right treatment in a manner in which they can understand. a coordinated relationship is maintained between the individual patients and their respective physicians. the five main attributes of PCMH are - they are patient-centered, comprehensive and coordinated care is given, they take care of the quality and safety, and their services are accessible.

PCMH model was originally introduced in the year 1967 by the American Academy of Pediatrics. later in the year 2007, this model was refined into a set of principles by the American Academy of family physicians, American academy of pediatrics, American college of physicians, and American osteopathic association.

Currently, PCMH has a great impact on the lives of the people as it has treated complex chronic illnesses. this is ensured that the care is patient-centered. now there is less waiting time for the physician's appointment, you can access 24/7 the medical care, faster test results, patient satisfaction, decreased incidences of emergency rooms because preventive care is also given, etc. Accreditation indicates additional recognition to the patients for a higher quality of care and safer practices. it offers cost-effectiveness as a major advantage because as per a comparative study, formerly there were 31000 privately insured patients in PCMHs but later on, there were over 350,000 non-PCMH patients in New York City.

the future role of PCMH is to encourage you to take a step forward for yourself in maintaining and improving your health. instead of getting paid for each and every service they provide, they ensure patient satisfaction and higher quality.
