That's a yes yes kind answer, that means the individual as well as the community is responsible depending in the situation and the type of health issue and the extend of health issue etc.
The patient r the individual knows the health hazards of smoking, but he ignores it and nurtures a habit of smoking , and may get diagnosed with lung cancer and suffer . Here we can not blame the community, because it was the patient's decision to be smoker.
Smoked foods , even though has many hazards, some communities accepts it as a life style and people suffer the consequences like Cancers of colon, stomach s etc.
The community supports vaccination of children, and people follow as the community. This prevents many diseases and the young generation lives a healthy life. Here the community is the reason of health promotion.
The individual goes in for own vegetable gardening and avoid pesticides, looking for fully organic vegetables. This results in improvement of his and his family health , with low risks for diseases. It was the person's decision to have a healthy life.
So even though the community puts up healthy ideas, individuals must be able to capture the essence of those ideas and put it to action.
