#. BPH - It is prostate enlargement as a function of age. It's a common occurrence in males particularly as age
The reason for this is that there is a bladder outlet obstruction from a benign enlargement of prostate it slows down to urinary flow
#. Risk factors of BPH :-
-age
-family history
-race-highest in african americans and lowest in asians and native americans
-diet high in meats and fat
#. Pathophysiology of BPH :-
-tissue obstructs urethra and bladder outlet
-urine flow becomes weak
-straining to empty bladder causes hypertrophy of bladder wall
-trabeculation of bladder wall leads to decreased bladder capacity
-increased filling pressure causes back up of urine into the kidney
-hydro ureters and hydronephrosis and kidney atrophy
-residual urine becomes alkaline from stasis- increases risk of infection and risk of stones
-promotes UTI's and renal and bladder calculi
#. Signs and symptoms of benign prostatic hypertrophy :-
-frequency urgency
-nocturia
-Burning on urination
-diminished force and flow of urine
-difficulty starting to flow of urine
-sensation of incomplete bladder emptying which causes urinary retention
-Post void dribbling
-increased bladder pressure
#. Health promotion :-
Individuals need to avoid drugs that cause urinary retention, Especially anti cholinergic's, antihistamines, and decongestants
#. Complications of benign prostatic hypertrophy :-
Obstruction can cause acute urinary retention resulting in
-dilated ureters
-hydronephrosis which can lead to kidney damage
-urinary tract infections
-bladder spasms
-bladder pain
#.Labs that would be ordered for a patient with BPH :-
-a urinalysis to check if infection is present. Specimen will contain white blood cells red blood cells and moderate bacteria
-CBC which is used to evaluate for any evidence of infection. Elevated WBC or anemia decreased RBCs
-B UN and creatinine levels to evaluate renal function; both are elevated with renal disease
-PSA which is also known as prostate specific antigen if prostate cancer suspected it will be elevated
-serum acid phosphatase levels
#. Diagnostic procedures for a patient with BPH :-
-KUB which is the next through the kidneys, ureters and bladder
-urodynamic flow studies to assess activity of the bladder and the bladder neck outlet during the emptying phase of voiding
-urography: useful studying both the structure and function of urinary tract
-Cystouretrhoscopic Examination
-amount of residual urine obtained by bladder scan: during this, the physician Bewes the interior of the bladder, the bladder neck, and the urethra
