Q: Think of an experience you have had with healthcare or one of your family or friends. Apply IOM’s six aims for improvement to the experience and identify how the experience fared according to the aims and the opportunities for improvement.

(IOM’s six aims is: 1.Safe, 2. Effective, 3. Efficient, 4. Timely , 5. Patient centered, and 6. Equitable)

I have an experience of my own aunty who was suffering from cancer but was not diagnosed from any of the private hospitals she went and she was given supportive care . Then her family members shifted her from the private hospitals to a government hospital due to their financial crunches . But the service that were provided by the government hospital were not that efficient. I will explain what happened there , the movement they reached the hospital in the ambulance she was on oxygen support cylinder ,there was no availability of stretcher to shift the patient and then after 20 -30 mins of waiting we arranged the stretcher and shifted her to the emergency . Then she was stcaked in the line towards Emergency. It took hours to get into the emergency . All this time we were requesting the doctors to see her once but they haven't until she gets in . Then suddenly she started having breathing difficulty and collapsed ,at the point when she crashed doctors and nurses run and came with emergency trolley and shifted her to HDU ,got ventilated , put CVP lines and started on ionotropes. She was constantly becoming hypotensive and was not improving and collapsed and died.

Here , according to me the main cause behind the death of my aunty is the provision of management care was not given timely , efficient ,effective ,safe ,patient centered and equitable . None of the IOM 's six aims were followed . If the services were provided according to the IOM's six aiims from the initial movement she won't have developed severe complications and collapsed.
