INTRODUCTION

Sample incident :-

An employee from the forest management company was electrocuted . One day the worker of the company who was monitoring the move of a supersnorkel which was loaded on a low bed trailer being towed by an off-highway tractor. The tip of the supersnorkel made contact with a high-voltage power line unnoticed by the worker. After he board the tractor , he placed his hands on the tractor which made him electrocuted fatally.

Steps conducted :-

• Investigation of the incident :- An investigation team has arrived and made an through investigation on the incident made .It was initially reported that the equipment energized from power line contact. Neither the truck driver nor the worker has noticed the supersnrokel  contacting with the power line. It was also investigated that the limits of the approach were violated .
• Identification of direct and root causes :- If we clearly understood and investigate ,on several occasions before this incident they have taken this supersnorkel in similar way and moved out of the yard .But this time the worker has set an angle which remained the core reason for the incident .The truck driver has depended upon the past experience of the worker.
• Disclosure of findings :- 1) Worker has set the angle 2) They have violated the height norms 3)Worker was little bit of relaxed of his past experience of handling the same 4) High voltage power line resulted in electrocuted fatally.
• Development of a corrective action plan :- From the next time on wards they should keep the equipment as low as possible and prevention of such incidents would be possible with true vigilance .A safety officer has to be employed to have initial checkup before the equipment moves to the desired destination.
• Implementation :- Corrective action plan has to be implemented as soon as possible because , the new workers will also have some same kind of fear . So , the management has to build confidence and trust among the workers by implementing the action plan soon.
• Follow-up review and revision :- The management  has to review the corrective plan again and again until the process become trust-worthy

• In the U.S they have to notify to OSHA(OCCUPATIONAL SAFETY AND HEALTH ADMINISTRATION)
• Coping with grief
• Family members should be informed first
• Body has to be post-mortemed
• Insurance coverage should be given (if applicable)
• Police investigation has to be done
• Compensation should be given as per the company rules and regulations
