Nondisjunction is, during cell division, when sister chromatids fails to segregate properly.

Incase of klinefelter syndrome, an extra chromosome X can be obtained during any of the following three events.

1). Paternal meiosis I (sperm with both X and Y)

2). Maternal meiosis I (egg with XX)

3). Maternal meiosis II (egg with XX)

In 1st case, fertilization with normal egg (X), results in XXY offspring.

In 2nd and 3rd case, fertilization with normal sperm with Y chromosome results in XXY offspring

Since segregation occurs during anaphase, the above events occurs during this phase.
